This note template is primarily checkboxes and dropdown selections, which utilizes SP’s wonderful copy-from-previous feature to reduce time to complete. The first time I complete this note for a new client, it takes the most time, as everything must be manually added. However, subsequent notes usually take five minutes or less.

Bonus: I created this note to be both an intake note AND a progress note, to make things even easier! There is a spot to copy-paste from your intake questionnaire or to hand write any diagnostic assessment or intake narratives you’d like to include. In future notes, you will deselect this item so this volume of information isn’t on future notes.

My preference is to write very little narrative material, but you could easily add more spaces for that, should you want that in your own note.

NOTE: This note template has never been through an audit, but has been carefully designed and adapted based on feedback from others who have been audited by various insurance companies.
